Bloom Advises Primary Care Group in Partnership with Agilon Health

The Bloom Organization (“Bloom”) announced that its clients, Syracuse New York based Family Care Medical Group (“FCMG”) and Practice Resources, LLC (“PRL”) have partnered with Agilon Health (“Agilon”), a portfolio company of global private equity manager Clayton, Dubilier & Rice (“CD&R”).  The venture is focused on value-based care and Medicare Advantage, and will modernize FCMG and PRL through technology, analytics, and population health tools.  FCMG is the dominant primary care group in the Syracuse, NY market.
